Forms available to request relating to the electoral register

Below are the types of forms that are available to request via the website. If the below do not meet your requirements please use the contact details at the bottom of this page to contact the elections team who will try to help.

Change of name form

A change of name form is used to update your name on the electoral register if it has legally changed. This could be due to marriage or by dead poll for example.

Confirmation of Registration Letter

A Confirmation of Registration letter can be used to confirm your current entry on the electoral register verifying how long you have been registered to vote at a certain address. We are unable to provide confirmation of registration letters for previous addresses.

Voter Authority Certificate

Voter Authority certificates are a free piece of photo identification which can be used to enable electors to vote in person if they have no other acceptable types of Photo ID. You can apply for a VAC online at Gov.uk - Apply for photo ID to vote or you can download a paper form at Gov.UK - Apply for a voter authority certificate by post if you're living in the UK .

Proxy vote application

A proxy vote allows you to appoint another registered elector to cast your vote for you so that you do not have to go to the polling station yourself.

Postal Vote Application

A postal vote is used to enable you to cast your vote before polling day if you cannot get to the polling station.

Postal Vote with Signature Waiver Application

A postal vote with a signature waiver is available for people who are either unable to physically sign or who are unable to provide a consistent signature. Your Postal vote will arrive at the same time as a regular postal vote, but it will only require yourself or someone you trust to help fill in the date of birth section. The area that would normally ask for a signature will say 'Waiver Granted'.

Postal Proxy Vote

If you have been appointed to act as a proxy for another elector, then you can cast that vote by post if you are unable to get to their polling station on polling day. This would work the same as a regular postal vote that would get sent to the proxy in the weeks before an election giving them time to complete and return.
